Annual forms are completed at the beginning of each school year by all families. This includes:
Parent/Guardian Contact information

Student Information Cards (Preferred Name [given or nickname], favorite colors/hobbies/bands/etc.) - This is a paper form to be filled out by students with parental/guardian help.
Grade-Specific Forms are sent home at various times throughout the school year, and will be dependent on the age of your child(ren)

Photo Release Forms:
Photo Release Forms are sent home in the event that Mrs. Morneau will be in need of utilizing photos/videos your child(ren)'s work and/or face for professional use only*. These photos are for Mrs. Morneau's professional teaching portfolio and are solely used for evidence of student learning. All images/videos and consent forms will be given to the family for consideration before they are used in the portfolio.
*Please note: If you have completed a Consent to Publish form through the District for your child(ren), that permission does not carry over for Mrs. Morneau to utilize photos in this manner and the Google Form will need to be completed.*
